CMS-4208-F2 & the REAL Health Providers Act

Medicare Advantage directory maintenance has moved from a low-priority back-office task to a major enrollment and financial risk. Historically, directories were plagued by "ghost networks" — retired, moved, or non-existent providers — with error rates exceeding 50%. These two rules are why that no longer flies.

CMS-4208-F2 — The Submission Pipeline

Direct CMS Submission

Beginning Plan Year 2027, all MA plans must submit their provider directories directly to CMS to populate the consumer-facing Medicare Plan Finder (MPF) tool.

Machine-Readable Format

Data must be made available via a publicly accessible, machine-readable JSON format conforming to the Da Vinci PDex Plan-Net HL7 FHIR-aligned schema specifications.

Suppression Risk

If a plan's directory file fails syntax validation during the daily CMS crawl, or executives fail to submit the annual accuracy attestation, CMS can suppress the plan from the Medicare Plan Finder — halting new enrollment, especially damaging during AEP.

The REAL Health Providers Act — Data Quality Framework

90-Day Verification Clock

Plans must actively reach out and verify the accuracy of every provider record at least every 90 days.

The "Unverified" Public Tag

If a provider is not verified within 90 days, the plan must prominently display a "data unverified" indicator tag on that provider's public directory listing.

5-Day Network Exit Window

If a provider leaves a network, the plan is legally required to remove them from all online directory listings within five business days.

Direct Cost Liability

Under surprise billing rules, if a member relies on an inaccurate directory listing and sees an out-of-network provider, the health plan is legally forced to absorb the difference in out-of-network costs.

Public Accuracy Scores (Plan Year 2029)

CMS will calculate and publish an official "Directory Accuracy Score" for every plan, which plans must display directly on their public sites.
